Additional new patient registration

This is the second part of the new patient registration. Please ensure that you have also completed the first part at NHS Register for a GP Surgery: Aldborough Surgery.

You can use this service if you:

• want to register at the surgery
• live within our catchment area
• have used the NHS Register for a GP Surgery: Aldborough Surgery service

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• your NHS number
• details of previous GP surgery
• basic health and medical information
